Job Description

We're seeking dedicated Psychiatric Social Workers to join our team in New Hampshire. In this role, you'll provide vital mental health services and psychosocial support to individuals and families facing mental health and substance abuse challenges. You'll assess client needs, develop personalized treatment plans, and offer therapeutic interventions to help them navigate crises and improve their overall well-being.

What You'll Do:

Conduct thorough psychosocial assessments to understand clients' mental health needs.
Develop and implement individualized treatment plans in collaboration with healthcare teams.
Build and maintain strong therapeutic relationships with patients, guardians, families, and others to assess, mobilize, and access resources that promote recovery.
Provide individual, family, and group therapy, utilizing a range of established therapeutic techniques.
Collaborate with the treatment team, patients, guardians, families, and others to address discharge planning and other important issues.
Advocate for clients to ensure they receive the appropriate resources and support.
Coordinate with other healthcare providers to deliver comprehensive and integrated care.
Offer crisis intervention services during psychiatric emergencies.
Educate clients and their families about mental health conditions, treatment options, and available resources.
Maintain accurate and confidential client records.
Facilitate access to community resources and support services.
Monitor and evaluate client progress, adjusting treatment plans as needed.
Oversee or assist with patient transportation services.
Initiate or oversee guardianship and/or involuntary commitment proceedings when necessary, adhering to ethical guidelines.
Support and assist other staff in providing courteous and clinically appropriate patient interactions.
Provide clinical analysis and recommendations at diagnostic and treatment review conferences.
Assist in covering social service needs as they arise.
What You'll Bring:

A minimum of one year of clinical experience in direct patient care as a Licensed Clinical Social Worker.
Experience in a variety of settings, such as detention or correctional healthcare, residential healthcare, hospitals, outpatient clinics, nursing homes, or other supervised medical facilities.
Experience working with both pediatric and adult patients.
A Master's degree in Social Work (MSW) from an accredited program.
Current licensure in New Hampshire with no record of disciplinary action.
Basic Life Support (BLS) Certification from an approved organization (AHA, Red Cross, ASHI).
Ability to read, write, and speak English effectively.
U.S. Citizenship or Permanent Resident status.
Reliable commute to Concord, NH or willingness to relocate.
Flexibility to travel on short notice or serve in various medical units.
